The Secret Service's investigation into the July 13 assassination attempt against former President Trump concluded that certain agents' actions may warrant discipline.

The new report largely echoed the findings of other investigations that have faulted poor communication between the Secret Service and local law enforcement helping out that day and the agency's failure to prevent nearby vantage points.
